Release of Liability and Consent Form * The purpose of this document is to (1) limit the liability of the Gene Potts Memorial, a Texas nonprofit corporation, and (2) provide Volunteers and Participants with guidance, safety rules, and instructions on how to have a fun and safe time while participating in or volunteering with the Gene Potts Memorial activities, including the Get Kids Cooking program.
The following procedures are asked to be followed at all times by all volunteers and participants:
1. All children aged 4 to 12 years old are eligible to participate with Get Kids Cooking as a participant, provided their parent or legal guardian signs the Release of Liability at the bottom of this document.
2. Only adults aged 20 and older who have successfully passed a background safety check will be allowed to volunteer with Get Kids Cooking. All participants’ parents are encouraged to volunteer!
a. Parents or guardians should assist their participating child with starting the fire, trimming with a knife, and ensuring the meat is cooked to the correct temperature for consumption.
b. The participants must be primarily responsible for seasoning, preparing, cooking the dish to taste, and presenting their dish.
c. In the event of excessive adult involvement in the handling of the prepared food, Participants may be disqualified from competing.
3. ALL VOLUNTEERS AND PARTICIPANTS ARE EXPECTED TO ABIDE BY REASONABLE STANDARDS OF SAFETY TO AVOID THE RISK OF INJURY. IF YOU ARE UNSURE WHAT CONSTITUTES REASONABLE STANDARDS OF SAFETY OR FAIL TO ABIDE BY SUCH STANDARDS, THEN YOU AND YOUR CHILD WILL NOT BE ALLOWED TO PARTICIPATE AND MAY BE REQUIRED TO LEAVE THE PREMISES.
4. Get Kids Cooking will supply the meat for the cook-off.
a. Each participant will select their proteins from the available options at the start of
the event.
b. The meat must be prepped and cooked within the designated cooking area
provided and within the time limit placed upon the event.
c. Any outside meat brought in by the participant or parent will result in
disqualification.
5. The following rules shall apply to the provision of equipment and supplies:
a. Each participant shall bring their own seasonings, cooking utensils, cooking source, and any other necessary equipment needed to get kids cooking.
b. We will provide a designated cooking space in the Get Kids Cooking Section, subject to availability. Cooking space may be available on a first come first served basis in situations where the sponsor’s dedicated event space is limited.
c. Participants may bring their own canopy, tables, chairs, extension cords or surge suppressors for their cooking space.
d. A fire extinguisher is required to be present at each cooking station.
6. Each child participant must have adult supervision present at their cooking space AT ALL TIMES throughout the event. If your child is participating, you should be present with your child in the cooking space when your child is in the cooking space.
7. All Get Kids Cooking events will begin with a mandatory Cooks’ Meeting. Any Participants or Volunteers who do not attend the Cooks’ Meeting shall not participate in the event.
8. During the Cooks’ Meeting, all participants will randomly select a number to determine the order in which they choose their protein to cook.
9. Additional event rules may be disclosed during the Cooks’ Meeting, such as plate turn-in procedures or unique event specific challenges.
10. Each competitor is responsible for keeping their cooking area clean during and after the event.
11. Prior to judging, all participant dishes shall be placed in the turn-in box.
a. Only the provided meat and a foil disc are allowed in the turn-in box.
b. No additional items or foreign objects are permitted. Violation of this rule will
result in disqualification.
c. “No additional items or foreign objects” refers to any items that are not directly related to the provided meat and its preparation. This includes but is not limited to strings, skewers, toothpicks, decorative picks, or any other objects that are not an integral part of the protein itself or its presentation.
12. Failure to meet the minimum number of proteins in the turn-in box will result in disqualification.
13. The turn-in container must not have any unidentified marks or labels on it.
14. Late turn-in of dishes will lead to disqualification.
15. Proper disposal of waste, including food scraps and packaging, must be followed and is subject to the direction of the event sponsor and/or the event volunteers and judges.
16. Get Kids Cooking participant dishes will be judged by an impartial panel of Volunteers
based on the following criteria and restrictions:
a. Blind Judging: all identifying material shall be absent from all dishes so that
judges do not know which participant prepared which dish.
b. Appearance: Presentation and visual appeal of the dish.
c. Texture: Evaluation of the meat’s tenderness and consistency.
d. Taste: Assessment of flavor profile and seasoning.
e. Overall Impression: Overall satisfaction and impression of the dish.
17. Participants will present their initials-marked ticket during awards or provide a photo of their ticket if their number is called.

I UNDERSTAND AND HEREBY EXPRESSLY ACKNOWLEDGE that I and my child who is participating in the event(s) hosted by the Gene Potts Memorial and Gets Kids Cooking may be asked to perform activities that are inherently dangerous to my me and my child, such as working with sharp instruments, using fire, working with heated cooking instruments, and other related hazards present in any outdoor kitchen. Further, I understand and hereby expressly acknowledge that these activities might, under some circumstances, pose certain health-related risks.
I HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO BRING SUIT against Gene Potts Memorial, its officers, directors, board members, agents, servants, employees, assigns, or successors, for any and all liability, claims, demands, actions, or causes of action arising out of any damage, loss, or injury to my person, employees, my property, whether or not resulting in injury or death, in the activities contemplated by this RELEASE, whether such loss, damage, or injury is caused by (1) defective equipment or (2) the negligence of Gene Potts Memorial, myself, or from some other cause. I hereby assume full responsibility for and risk of bodily injury, death or property damage that I suffer while performing the above-stated activities.
ON BEHALF OF MY MINOR CHILD, I HEREBY WARRANT AND PROMISE THAT I AM MY CHILD’S LEGAL GUARDIAN AND NO OTHER PARTY IS REQUIRED TO SIGN THIS RELEASE.
ON BEHALF OF MY MINOR CHILD, I RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO BRING SUIT against Gene Potts Memorial, its officers, directors, board members, agents, servants, employees, assigns, or successors, for any and all liability, claims, demands, actions, or causes of action arising out of any damage, loss, or injury to my child’s person or property, whether or not resulting in injury or death, in the activities contemplated by this RELEASE, whether such loss, damage, or injury is caused by (1) defective equipment or (2) the negligence of Gene Potts Memorial, myself, my child, or from some other cause. I hereby assume full responsibility for and risk of bodily injury, death or property damage that I suffer while performing the above-stated activities.
GENE POTTS MEMORIAL DOES AGREE TO HOLD HARMLESS ALL Volunteers and Participants in any event hosted by the Gene Potts Memorial for any disputes related to their activities as a Volunteer or Participant, to the extent that such Volunteers and Participants follow the rules of the event as herein described.
I UNDERSTAND that I may decline to participate in any event I deem too hazardous for me or my child. Consent to participate is completely voluntary and may be discontinued at any point. I understand it is solely my responsibility to determine what is too hazardous for me or my child.
ADDITIONALLY, BY CONSENTING TO PARTICIPATE IN AN EVENT HOSTED BY THE GENE POTTS MEMORIAL, OR GET KIDS COOKING, I CONSENT to follow the rules of the event as herein described and to follow the instructions of the Volunteers and event coordinators.
By signing this form, I am consenting to participate in the activities stated above and I am releasing Gene Potts Memorial from liability from any injury that might occur as a direct result from my participation or my child’s participation.
